Yeah I have the same problem. It happens with any game I play, new or used.
It will work fine for a different amount of time each day, pop up the error. I have to shut it down and turn it back on, or get up and pop the disc tray open and close it and it will work fine the rest of the day, or how ever long I'm playing.
It's just a little bit irritating when you lose all your progess in a game due to this.
I have had it happen once on PDZ and like half a dozen on GoW, always on single player never online. It always goes away if I open and close the disc drive. I think theres like a piece in there that is badly made, which is why there is excessive noise vs the ps3.
Mine will occasionally say unreadable disk, but I will turn mine vertical, or horizontal (whichever was causing the problems) and it usually fixes itself. Saved me from sending mine in.

If you're not under warrenty anymore, pop open the Xbox (tutorials around the web) and then pop open the Dvd drive case, and clean the lens with a little bit of rubbing alochol and q-tip.
Ok, this is an oldschool technique. worked wonders with my PS2. get two pieces of scotch tape an inch long a piece. place the two pieces vertically in the center of the disc as close to the center hole as possible. sometimes the disc spins a little unevenly and needs to be balanced out. Might work, if anything it's worth a try.
If your under warranty call MS, if your not grab a thing of compressed air and blow it inside of the box with the disk tray open.
It worked for the old xbox and my dvd player so iam assuming it will work for the 360.
BUT whatever you do DONOT try one of those disk that supposedly clean the lense. I tried one of those and it managed to ruin my 3rd Ps2, its got little brushes that get rid of the dirt on the lense but all it seemed to do was scratch the lense up even more.
its actually wierd, if u have your power cord plugged into a surge protector then u will see this error alot. From what I heard is when u plug it into a protector the console doesnt work as fast which slows down the speed of the disc tray which will give u the disc read error. I never had a problem but a couple of my friends did, and this seemed to fix the problem. So try it if u want.
